My OneCare offers patients personalized and secure on-line access to view a summary of their past doctor's office or hospital visits. Visit information includes a summary of the condition that prompted the visit, along with any medications, allergies, adverse reactions or alerts.  With My OneCare, you can view a summary of your past visits any time that is convenient for you.  Your health information, including medication name and dosage amounts, allergies, and any adverse reactions are available at your fingertips.

Is there a fee to use My OneCare?
No.  My OneCare is a free service offered to our patients.
How do I sign up?
If you wish to participate, you can register during your doctor's office or hospital visit. You will be asked to create a secret code, and a message will be sent to your email address. The email invitation, along with the activation code and the secret code you create, will enable you to log in and create your own username and password.

It is necessary to request access in person to ensure the right medical record is applied to the correct person.  It is not possible to sign up over the phone.
Can my spouse and I share one My OneCare account?
No. Because of the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish his or her own My OneCare account.
